Important Requirements for CPM-MI's Preferred Vendors and Service Providers
To start, please head over to https://www.michigan.gov/leo/bureaus-agencies/wdca/forms/insurance/wc-337-notice-of-exclusion for information on workers comp laws and to download any necessary forms for exemptions.
Next, we'll need you to provide us with the following:
- A completed W-9 Request for Taxpayer Identification Number and Certification
- A copy of your Trade Name Registration from the Secretary of State
- A Liability Insurance Certificate from your Insurance Agent with a minimum of $1,000,000.00 in Liability Coverage
These requirements are essential in order for you to be approved as a vendor and receive payment for your goods or services. We can't wait to work with you and grow our community together!
